Job Overview:

Vibrant Hearts Home Care seeks a skilled Registered Nurse to serve as a Care Coordinator in our team. This role involves providing daily supervision of CHHA and PCT staff, ensuring the quality of service meets the required plan of care.

About the Role:

  • We are seeking a Registered nurse with a Bachelor's degree in nursing and experience in a home care setting.
  • You will be responsible for initiating and reviewing client medical history, medications, and other statuses from client records.
  • The ideal candidate will have excellent communication skills, both verbal and written, and be able to identify and evaluate client needs.
  • As a Care Coordinator, you will work closely with electronic medical records to ensure compliance with company standards.
  • You will also provide coaching support to new staff to ensure compliance and consistency.

Requirements:

  • Estimated salary: $70,000 - $90,000 per year
  • Registered nurse in good standing, holding a Bachelor of Science degree in nursing
  • Knowledge and experience in a home care setting
  • Ability to communicate in English well, both verbally and in writing
  • Bilingual (preferable)
  • Willingness to travel
