Comprehensive Care Coordinator

We are looking for a highly motivated Comprehensive Care Coordinator to join our team at Strive Health. As a critical member of our care team, you will be responsible for coordinating services to optimize care delivery for patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD).
This role offers a competitive salary range of $74,000 - $90,000 annually, with opportunities for growth and professional development. Our comprehensive benefits package includes medical, dental, and vision insurance, as well as 401k with employer match, and paid holidays and time off.
The ideal candidate will possess strong analytical and problem-solving skills, with experience in case management and nursing. Proficiency in Microsoft 365 Suite and electronic medical records (EMR) is also required.
Key Responsibilities:- Work closely with the care team to craft personalized care plans addressing clinical and non-clinical needs of patients
- Conduct regular assessments onsite and virtually to detect undisclosed medical or social issues and changes in the home environment
- Initiate proactive outreach to patients and/or clinics to evaluate their status and offer assistance
