About the Role

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Nursing Care Coordinator for pediatric patients. In this role, you will play a vital part in delivering high-quality care to children facing life-limiting illnesses and their families.

Responsibilities
  • Evaluate patients and create personalized care plans to address their physical, emotional, and spiritual needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care strategies.
Requirements
  • 3 years of experience as a nurse in a clinical care setting or home health environment.
  • 1 year of pediatric experience required.
  • Current state license as a registered nurse.
