Care Coordination Specialist

We are looking for a skilled Care Coordinator to join our team at Eventus WholeHealth. As a key member of our team, you will be responsible for ensuring that patients receive high-quality care and support throughout their healthcare journey.
About the Role:
This full-time position involves working closely with our interdisciplinary care team to manage client caseloads, coordinate primary and specialty care, and oversee transitions of care. You will also collaborate with patient schedulers to ensure timely follow-up appointments.
Responsibilities:
- Develop and implement effective care plans to meet the unique needs of each patient
- Collaborate with healthcare providers to ensure seamless communication and coordination of care
- Monitor patient progress and adjust care plans as needed
Educational Requirements:
A Licenced Practitioner (LPN) degree or equivalent certification and 2 years minimum experience in the LTC/ALF setting are required. Strong knowledge of geriatric population and patient navigation is essential.
$62,500 - $72,000 per year, depending on experience.
