Overview:

This role is designed for a Clinical Pharmacist specializing in Internal Medicine within the Pharmacy department at RWJBarnabas Health.

Key Qualifications:

Mandatory:

  • Active pharmacist registration in New Jersey; current licensure is essential.
  • Licensed pharmacist immunizer in New Jersey.
  • Graduation from an accredited School of Pharmacy with a Bachelor of Science in Pharmacy or PharmD.
  • Completion of a PGY1 residency program or equivalent professional experience.
  • Pharmacotherapy Specialty Certification (BCPS) to be obtained within the first year of employment.
  • BLS and ACLS certifications required within six months of starting.

Preferred:

  • Completion of a PGY2 residency in Internal Medicine or equivalent experience is preferred.

Work Schedule:

  • Full-Time
  • Day Shift

Core Responsibilities:

  • Deliver targeted pharmaceutical care through inpatient rounds and serve as a drug information resource for the Internal Medicine teaching service.
  • Provide ambulatory care diabetes services on a weekly basis in the Internal Medicine Faculty Practice.
  • Offer cross-coverage, patient counseling, and drug information support for various clinical pharmacy specialty services.
  • Enhance professional development through participation in educational programs, literature reviews, workshops, and in-service meetings.
  • Monitor medication prescribing in accordance with established criteria prior to dispensing.
  • Facilitate educational rotations for both PGY1 and PGY2 residents.
  • Support PGY1 and PGY2 resident research initiatives.
  • Consult and educate patients regarding their medications for both acute and chronic conditions.
  • Utilize a medication screening process to ensure completeness and appropriateness of medication orders.
  • Document and analyze all clinical activities, interventions, medication errors, and adverse drug reactions as per departmental criteria.
  • Engage in the Diabetes Stewardship Committee.
  • Participate in the Ambulatory Care Corporate Collaborative.
