Job Summary:
This role involves serving as a Clinical Specialist Pharmacist within the Pharmacy department, focusing on Internal Medicine. The position emphasizes the provision of pharmaceutical care and collaboration with healthcare teams to enhance patient outcomes.

Qualifications Required:
- Valid New Jersey pharmacist registration and licensure
- Certification as a licensed pharmacist immunizer in New Jersey
- Completion of an accredited School of Pharmacy program, holding a Bachelor of Science in Pharmacy or PharmD
- Successful completion of a PGY1 residency program or equivalent experience
- Pharmacotherapy Specialty Certification (BCPS) to be obtained within one year of employment
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required within six months of employment

Preferred Qualifications:
- Completion of a PGY2 residency in Internal Medicine or equivalent experience is preferred

Scheduling Requirements:
- Full-Time
- Day shifts

Key Responsibilities:
- Deliver targeted pharmaceutical care through inpatient rounds and serve as a drug information resource for the Internal Medicine teaching service.
- Provide ambulatory care diabetes management services weekly in the Internal Medicine Faculty Practice.
- Offer cross-coverage, patient counseling, and drug information support for various clinical pharmacy specialty services, including but not limited to Renal Transplant, Perioperative services, and Geriatrics.
- Foster professional development through participation in educational programs, literature reviews, workshops, and in-service meetings.
- Monitor medication prescribing in line with established drug usage criteria prior to dispensing, as outlined by the Pharmacy & Therapeutics Committee.
- Conduct educational rotations for PGY1 and PGY2 residents and facilitate their research projects.
- Provide consultations and educate patients regarding their medications for both acute and chronic conditions.
- Utilize the medication screening process to ensure completeness, appropriateness, and legal compliance of medication orders.
- Document and analyze all clinical activities, interventions, medication errors, and adverse drug reactions according to departmental criteria.
- Engage in the Diabetes Stewardship Committee and participate in corporate collaborative initiatives.
