Job Summary:
This role encompasses the responsibilities of an Internal Medicine Clinical Pharmacist within the Pharmacy department at RWJBarnabas Health. The position focuses on delivering exceptional pharmaceutical care and expertise in a clinical setting.
Qualifications:
Mandatory:
- Active pharmacist registration in New Jersey; current licensure is essential.
- Certification as a licensed pharmacist immunizer in New Jersey.
- Completion of an accredited Pharmacy program, holding a Bachelor of Science in Pharmacy or PharmD.
- Completion of a PGY1 residency program or equivalent professional experience.
- Attainment of Pharmacotherapy Specialty Certification (BCPS) within one year of employment.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required within six months of employment.
Preferred:
- Completion of a PGY2 residency in Internal Medicine or comparable experience is preferred.
Work Schedule Requirements:

Key Responsibilities:
- Deliver specialized pharmaceutical care through active participation in inpatient rounds, serving as a drug information resource for the Internal Medicine teaching service.
- Provide ambulatory care diabetes management services weekly within the Internal Medicine Faculty Practice.
- Offer cross-coverage, patient counseling, and serve as a drug information resource for various clinical pharmacy specialty services, including but not limited to Renal Transplant, Perioperative services, and Geriatrics.
- Foster professional development through engagement in educational programs, literature review, workshops, and in-service meetings.
- Monitor medication prescribing practices in line with established drug usage guidelines prior to dispensing, as defined by the Pharmacy & Therapeutics Committee.
- Facilitate educational rotations for both PGY1 and PGY2 pharmacy residents.
- Support PGY1 and PGY2 resident research initiatives.
- Provide consultations and educate patients regarding their medications for both acute and chronic disease management.
- Implement a medication screening process to ensure that medication orders are complete, appropriate, legally compliant, and prioritized.
- Document and provide analytical support for all clinical activities, interventions, medication errors, and adverse drug reactions according to departmental criteria.
- Participate in the Diabetes Stewardship Committee and the Ambulatory Care Corporate Collaborative initiatives.
