At WellBe Senior Medical, we are dedicated to providing a comprehensive care model led by physicians and advanced practice clinicians, specifically tailored for geriatric patients. Our focus is on serving the frail, multi-chronic elderly population enrolled in Medicare Advantage plans, who often face significant barriers to accessing healthcare services. We strive to deliver care directly to our patients, ensuring they receive the necessary support across all stages of their healthcare journey—from home-based chronic and urgent care to hospital services, skilled nursing facilities, assisted living, palliative care, and end-of-life support.

POSITION SUMMARY

As a Patient Care Scheduler, your primary responsibility will be to manage incoming calls from WellBe patients regarding appointment scheduling, modifications, or cancellations. You will ensure that all patient information is recorded accurately, adhering to confidentiality standards and HIPAA regulations. Additionally, you will route calls to the appropriate departments as necessary.

Key Responsibilities:

  • Coordinate member visit schedules in accordance with established guidelines, prioritizing patient needs while optimizing provider travel time.
  • Plan and execute appropriate scheduling for clinical providers based on visit requirements.
  • Utilize electronic health record (EHR) systems and other software applications to schedule appointments with a high degree of accuracy.
  • Adjust visit times as needed to enhance the availability of clinical providers.
  • Assess clinical provider availability and modify schedules to ensure optimal patient care.
  • Notify the operations manager of any scheduling conflicts or requests for changes from clinical providers.
  • Gather necessary information through appropriate channels to facilitate effective scheduling.
  • Confirm scheduling details with marketing staff to ensure accuracy and timeliness.
  • Effectively communicate any changes or cancellations to relevant clinical providers and departments.
  • Perform additional tasks as required to meet team objectives.

Qualifications:

Education and Experience:

  • High school diploma or GED is required.
  • A minimum of 2 years of experience in patient or customer scheduling is required.

Essential Skills and Abilities:

  • Familiarity with common medical terminology.
  • Exceptional communication skills and professional phone etiquette are essential.
  • Bilingual abilities (English/Spanish) are highly advantageous.
  • Experience with electronic medical records (EMR), particularly Greenway, is preferred.
  • Strong written and verbal communication skills are necessary.
  • Demonstrated ability to self-manage workload and collaborate effectively with team members.
  • A commitment to patient relationships and a focus on customer service and patient experience are crucial.

Supervisory Responsibilities: This position does not include supervisory duties.

Travel Requirements: Minimal travel may be required.

Work Environment: This role will be performed in various settings, including an office environment. The noise level is typically moderate, and the position requires prolonged sitting, as well as physical activities such as bending, stooping, and reaching. The ability to operate office equipment and communicate effectively via telephone or computer is essential. Candidates must be able to lift and/or move up to 25 lbs.
