Job Title: Social Worker

Job Summary:

We are seeking a skilled Social Worker to join our team at Northwell Health. The successful candidate will provide individual and group work services to patients and their families in various healthcare settings.

Responsibilities:

* Provide culturally competent direct services to patients and their families using a variety of social work modalities.
* Utilize high-risk criteria and consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention.
* Advocate on behalf of the individual to ensure maximum access to healthcare, assess barriers, and offer solutions.
* Provide interdisciplinary teams with assessment findings and suggest potential impact on overall patient care.
* Provide psychoeducation, counseling, and crisis intervention to patients and their families.
* Formulate and implement treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice.
* Initiate communication with post-acute providers and payers as needed.
* Incorporate knowledge of community resources and entitlement programs in all planning.
* Maintain appropriate professional patient care documentation as defined by site policies and procedures.
* Represent the department and profession in various hospital/site-based committees and work groups.

Requirements:

* Master of Social Work required.
* Current License to practice as a Master Social Worker or Clinical Social Worker in New York State required, plus specialized certifications as needed.
* 1-3 years of relevant experience required.

Additional Salary Detail:

The salary range for this position is $66,220-$108,180/year.
