Social Services Professional

Job Summary
Northwell Health is seeking a skilled Social Worker to provide support and resources to patients at the Speech & Audiology Center. The ideal candidate will assist department team members in follow-up care and collaborate with interdisciplinary teams to ensure comprehensive patient care.
Key Responsibilities
- Provide culturally competent services to patients and their families using various social work modalities.
- Identify patients in need of or who could benefit from intervention using high-risk criteria and/or consultation referrals.
- Advocate on behalf of patients to ensure maximum access to healthcare and assess barriers to care.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Provide psychoeducation and community resources to patients and their families.
- Maintain accurate and timely professional patient care documentation.
Requirements
- Master of Social Work degree required.
- Current license to practice as a Master Social Worker or Clinical Social Worker in New York State required.
- 1-3 years of relevant experience preferred.
