CareStar, Inc. is seeking a highly motivated and experienced Community-Based Case Manager to join our Clinical Department. As a key member of our team, you will play a vital role in assisting and enabling individuals to remain in the least restrictive environment while maintaining their independence and dignity.
Key Responsibilities:- Assist participants with access to and coordination of services to enable them to receive a range of appropriate services in a planned, coordinated, efficient, and effective manner.
- Promote and support participants' preferences, values, and the right to self-determination.
- Develop and implement care plans, including the authorization of the amount, scope, and duration of services.
- Conduct home visits and/or appropriate contacts and monitor services in compliance with CareStar Case Management and assessment expectations.
- Complete incident reports, submissions, and follow-ups using State-approved processes.
- Identify and report high-risk situations with consumers and take appropriate action to mitigate health risks.
- Active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
- 12 months of experience in home and community-based services within the past 5 years is preferred.
- Knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs is desirable.
