Job Title: Community Health Worker
Job Summary: CareStar, Inc. is seeking a Community Health Worker to conduct program assessments using program and CareStar identified tools and process. This is a home-based position that works independently to manage a caseload as needed.
Responsibilities:
- Conduct program assessments using program and CareStar identified tools and process.
- Manage a caseload independently.
- Adhere to the CareStar Rule in performance of job responsibilities.
- Understand and comply with CareStar Policies and Procedures.
- Maintain confidentiality as related to patient information.
Requirements:
- Active license as RN, LSW, or LISW is preferred.
- 12 months experience in home and community based services within the past 5 years.
- Knowledge of Medicare, Medicaid, home health care and Home and Community-Based waiver programs desirable.
