One of our clients is seeking School Case Manager in the South Philadelphia, PA area (up to $3200 monthly)

CLIENT'S AVAILABLE HOURS Monday through Friday 9am-4pm b>

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
A case manager has a variety of responsibilities, including:
" Assessing clients: Evaluating clients and referrals, and providing counseling on issues like child safety and parent behaviors
" Setting goals: Establishing goals and tracking progress towards them
" Connecting clients with resources: Helping clients access human services, such as medical, mental health, and income supplemental services
" Coordinating services: Working with a family's DHS worker to coordinate services like mental health, drug and alcohol, anger management, and housing
" Conducting home visits: Making monthly home visits to families assigned to the caseload
" Preparing reports: Completing family assessment forms, summaries of service, quarterly reports, court reports, and closing reports
" Communicating concerns: Immediately reporting concerns about child safety to the supervisor and Department of Human Services (DHS)
" Supporting family attendance: Preparing for and supporting families' attendance at center-based programs

CLIENT'S REQUIRED SKILLS & EXPERIENCE Bachelors in Human Services or Education.
1+ years' experience as a counselor, behavioral specialist, or mobile therapy.
Ability to complete and pass clearances: FBI, Child Abuse, PA State Background Check, & Act 126.
Provide documentation of: CPR Certification, First Aide Level 2 Certification.
Must be able to produce current medical/health documents including: Physical examination, Tuberculosis (TB) test, Hepatitis B screening.
