About the Role:

We are seeking a highly skilled and experienced Case Management Lead to join our team. As a Case Management Lead, you will play a critical role in helping our organization achieve its mission of providing quality services to individuals with complex needs.

Key Responsibilities:
  • Provide case management reviews and supervision to ensure high-quality services are delivered to our participants.
  • Oversee the development and implementation of service plans for individuals served, ensuring that their needs are met and their goals are achieved.
  • Monitor adherence to treatment plans and ensure that all necessary documentation is completed in a timely manner.
  • Supervise assigned staff, including performance evaluations, scheduling, orientation, training, and proper certification.
  • Investigate internal incidents as required and provide support to employees in a management capacity.
  • Participate in community resource development, attend all required meetings and engagements, and contribute to quality assurance planning and implementation.
Requirements:
  • Master's degree in social work or human services field required.
  • At least 2 years' experience working in child welfare.
  • At least 1 year supervisory experience.
  • Reliable, responsible, and caring nature.
  • Exceptional attention to detail with the ability to multi-task.
  • Ability to work well with others and build rapport.
