About the Unit

Our sub-acute floor is a 59-bed Medicare unit that caters to residents with complex clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related conditions requiring close monitoring. Nurses collaborate with a multidisciplinary team, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support, to provide comprehensive care.

Our Chronic Care Floor

The 60-bed Medicare/Rehabilitation and long-term care unit offers a dynamic environment for learning and professional growth. Nurses work closely with healthcare professionals to develop and implement effective treatment plans for residents. This fast-paced unit provides a great atmosphere for PT/OT residents to achieve their goals.

Our Dementia Care Floor

We provide care to residents with various diagnoses on our 60-bed Medicare/Medicaid/Dementia unit. Our team creates a fullness of life through relationships, the art of caring, and the spirit of living. We work closely with a multidisciplinary team to discuss and implement resident care plans.

About the Facility

Beaumont Commons – Farmington Hills is a 3-floor facility with 9 units, providing care for long-term care residents and post-acute rehab patients. Our first floor has 59 beds focused on sub-acute rehab, the second floor has 59 beds focused on long-term care and sub-acute rehab, and our third floor is a secured dementia unit. Nurse-patient ratios vary depending on the shift and population served.

Scope of Work

The Social Service Worker provides and/or coordinates medically related social services to attain and maintain the highest practicable physical, mental, and psychosocial well-being of each resident. This is achieved through the use of appropriate assessment tools, development, completion, and revision of resident care plans, and maintaining contact with residents and their family members.

Qualifications

Required: Associate's Degree in social work or a human services field. Preferred: Bachelor's Degree in Social Work (BSW) or LIC-Bachelor Social Worker (BSW) in the State of Michigan. Continuing care is essential for this role.
