Social Worker BSW Long Term Care Specialist

Our sub-acute floor is a 59-bed Medicare unit that provides care to residents with multiple clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related issues requiring monitoring.
Nurses work closely with a variety of healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support, to provide comprehensive care to our residents.
Our Sub-Acute UnitWe offer a well-rounded opportunity for learning, professional growth, and personal satisfaction on our sub-acute unit.
Our Chronic Care floor is a 60-bed Medicare/Rehabilitation and long-term care unit, providing a great place to learn basic med-surg, I.V. therapy, and wound treatments nursing skills.
Nurses on this unit work with a host of healthcare professionals to provide and implement the best treatment plan for residents.
This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
About the FacilityBeaumont Commons – Farmington Hills is a 3-floor facility with 9 units, providing care for long-term care residents and post-acute rehab patients.
The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds focusing on long-term care and sub-acute rehab, and our third floor is a secured dementia unit.
Nurse-patient ratios vary depending on the shift and the population you will be serving.
We care for dementia residents, long-term care, and sub-acute rehab residents, with specialties including general rehab, neuro rehab, cardiac, respiratory, and cancer.
We also have various programs for residents to participate in multiple activities.
Scope of WorkThe Social Service Worker provides and/or coordinates medically related social services to attain and maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
This is achieved through the use of appropriate assessment tools, development, completion, and revision of resident care plans, and maintaining contact with residents and their family members.
The Social Service Worker ensures that all charted progress notes are informative and descriptive of the services provided, including the resident's response to the service.
