We are seeking a highly skilled and compassionate Patient Navigator, Surgery to join our team at Hackensack Meridian Health. As a Patient Navigator, you will play a vital role in ensuring that our patients receive the highest level of care and support throughout their surgical journey.
Key Responsibilities- Treat patients with dignity and professionalism, providing competent clinical care during office visits.
- Facilitate communication between patients and healthcare providers, ensuring seamless coordination of care.
- Provide education and resources to patients and their families, empowering them to make informed decisions about their care.
- Greets patients courteously, efficiently processes their information, and completes physician's orders accurately and thoroughly.
- Guides assigned patients through the healthcare system, advocating for their needs and preferences.
- Supports patients and families in coping with the psychological aspects of cancer, connecting them with resources and social support systems.
- Coordinates appointments with providers, ensuring timely delivery of diagnostic and therapeutic services.
- Answers patient telephone calls, processing information for appointment scheduling, answering questions, and assisting with problem-solving.
- RN, (Associates, Diploma, BSN) degree required.
- Minimum of 3 years of nursing experience in related field (Surgery/Oncology) required.
- Good computer skills and excellent written and verbal communication skills required.
- Strong organizational and critical thinking skills required.
- NJ State Professional Registered Nurse License required.
