Key Responsibilities:
- Advocacy and Education: Serve as a patient advocate and educator, providing guidance and support to patients and their families throughout the healthcare continuum.
- Clinical Care Coordination: Coordinate patient care by facilitating communication between patients and healthcare providers, ensuring that patients receive timely and appropriate treatment.
- Resource Navigation: Connect patients with resources and social support systems, helping them to access the services they need to manage their care.
- Community Engagement: Participate in community education and outreach efforts to raise public awareness about healthcare services and promote healthy behaviors.
- Collaboration and Communication: Work closely with healthcare providers, investigators, and other members of the clinical research team to identify patients as potential research subjects for available clinical trials.
Requirements:
- Education: Nursing Diploma or ASN
- Experience: Minimum of 3 years of nursing experience in a related field (Surgery/Oncology)
- Certifications: NJ State Professional Registered Nurse License and BLS-AHA
