Hackensack Meridian Health is seeking a skilled Patient Navigator, Surgery to join our team. As a key member of our healthcare team, you will play a vital role in ensuring our patients receive the highest level of care and support throughout their surgical journey.

Responsibilities
  • Provide exceptional patient care and advocacy, ensuring patients and their families feel supported and informed throughout the surgical process.
  • Collaborate with healthcare providers to coordinate patient care, ensuring timely and effective delivery of diagnostic and therapeutic services.
  • Develop and implement personalized care plans, taking into account patients' unique needs and circumstances.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and minimize delays.
  • Identify and address potential barriers to care, connecting patients with necessary resources and support systems.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care.
  • Stay up-to-date with the latest clinical research and guidelines, applying this knowledge to improve patient outcomes and experiences.
Requirements
  • RN degree (Associates, Diploma, BSN) with a minimum of 3 years of nursing experience in a related field (Surgery/Oncology).
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Good computer skills and proficiency in electronic health records (EHRs) systems.
