Job Summary

The Case Manager RN is a vital member of our healthcare team, responsible for coordinating comprehensive care for patients across the continuum. This role requires advanced nursing skills and knowledge to ensure seamless transitions and optimal patient outcomes.

Key Responsibilities
  • Advocate for patients and families throughout the care process
  • Monitor patient progress and intervene as needed to ensure efficient care
  • Coordinate discharge planning and establish safe discharge plans
Requirements
  • BSN degree
  • 2 years of medical surgical nursing experience
  • Preferred: 1-2 years of acute care, homecare, or population health RN case management experience; CCM certification
