Position Summary:

We are currently looking for a Vocational Support Specialist to deliver continuous vocational and rehabilitative assistance to individuals facing mental health challenges such as psychosis, depression, and anxiety. The primary objective of this program is to mitigate or avert functional decline and aid individuals in securing employment or educational prospects.

This role is part-time and funded through grants, with annual renewal possibilities.

The selected candidate can anticipate working between 10 and 25 hours weekly, with some flexibility for remote work.

Our organization fosters a collaborative environment with adaptable work hours, encouraging teamwork with various professionals, including peer support specialists, education and employment advisors, outpatient therapists, case managers, and psychiatrists.

Key Responsibilities:

  • Consistently address the vocational needs and aspirations of individuals in your care.
  • Oversee and ensure compliance with all documentation related to the support provided.
  • Travel to various work sites to facilitate sustainable job experiences for clients.
  • Implement the Individual Placement and Support (IPS) model for youth and young adults.
  • Provide effective vocational assistance using the Coordinated Specialty Care framework.
  • Develop and execute treatment plans aimed at advancing clients' vocational and educational objectives.
  • Complete timely clinical documentation in the Electronic Health Record (EHR) that meets agency, federal, Medicaid, Managed Care Organization (MCO), and licensure standards.
  • Administer data collection tools for all youth served at designated points.
  • Participate in youth and family team meetings as necessary, coaching individuals on self-advocacy.
  • Document all services, interactions, and attempts to contact clients, their families, and relevant parties.
  • Maintain professional boundaries with clients and their families.
  • Attend all required county and state training sessions, participating in individual and group coaching as needed.
  • Carry a work phone to provide both telephone and face-to-face support.

Qualifications:

  • 1-2 years of experience in providing vocational services within a human services context.
  • Proficient in Microsoft Suite and EHR systems.
  • Bilingual and multilingual candidates are encouraged to apply.
  • One year of experience working with individuals facing mental health challenges is required.
  • A valid driver's license and reliable personal transportation are necessary.
  • 3 years of experience serving youth and families with complex mental health, substance use, and co-occurring disorders.
  • Experience in participating in Coordinated Specialty Care interventions or school-related meetings (IEP, 504).
  • Proven ability to collaborate with families, natural supports, and community stakeholders.
  • Experience in maintaining clinical documentation for client records, including treatment plans, progress notes, quarterly reviews, and annual assessments.

Educational Requirements:

A minimum of a high school diploma or GED is required.

Preferred Education:

A bachelor's or master's degree in rehabilitation, counseling, social work, nursing, or a related field in human services is preferred.
