Role Summary:

We are currently seeking a Vocational Support Specialist to deliver ongoing vocational and rehabilitative assistance to individuals facing mental health challenges, including conditions such as psychosis, depression, and anxiety. The primary objective of this program is to mitigate or avert declines in functional outcomes while aiding individuals in securing employment and educational opportunities.

This position is part-time and is funded through grants, with annual renewal considerations.

The selected candidate can anticipate working between 10 and 25 hours per week, with flexible telework options available.

Our organization fosters a collaborative team environment, offering flexible working hours and encouraging cooperation with other professionals, including peer support specialists, education and employment advisors, outpatient therapists, case managers, and psychiatrists.

Ideal Candidate Profile:

The successful candidate will be passionate about working consistently with individuals facing mental health challenges, supporting their growth and achievement of personal goals over time. They will also be committed to facilitating access to various aspects of career and education with respect and support.

Key Responsibilities:

  • Consistently address the vocational needs and aspirations of individuals on your caseload.
  • Ensure compliance with all documentation related to the supports provided.
  • Travel to a variety of work sites to help maintain sustainable job experiences for clients.
  • Utilize the Individual Placement and Support (IPS) model for youth and young adults.
  • Provide effective vocational support using the Coordinated Specialty Care framework.
  • Develop and implement treatment plans to facilitate client progress in vocational and educational pursuits.
  • Maintain timely clinical documentation in Electronic Health Records (EHR) that meets agency, federal, Medicaid, Managed Care Organization (MCO), and licensure standards.
  • Administer relevant data tools for youth at designated collection points.
  • Participate in youth and family team meetings as appropriate, coaching individuals on self-advocacy.
  • Document all services, contacts, and attempts to reach clients, their families, and other stakeholders.
  • Maintain professional boundaries with clients and their families.
  • Attend all required county and state training sessions and participate in individual and group coaching as necessary.
  • Provide telephone and face-to-face support as needed.

Qualifications:

  • 1-2 years of experience in providing vocational services within a human services context.
  • Proficient in Microsoft Suite and EHR systems.
  • Bilingual and multilingual candidates are encouraged to apply.
  • One year of experience working with individuals facing mental health challenges is required.
  • A valid driver's license and reliable personal transportation are necessary.
  • 3 years of experience providing services to youth and families with complex mental health, substance use, and co-occurring disorders.
  • Experience in participating in Coordinated Specialty Care interventions or school-related meetings (IEP, 504).
  • Experience collaborating with families, natural supports, and community stakeholders.
  • Proficient in maintaining clinical documentation necessary for client records, including treatment planning, progress notes, quarterly reviews, and annual assessments.

Educational Requirements:

A minimum of a high school diploma or GED is required.

Preferred Education:

A bachelor's or master's degree in rehabilitation, counseling, social work, nursing, or a related human services field is preferred.
