Job Overview

Intermountain Deaconess Children's Services is a prominent non-profit organization dedicated to providing support and healing to children, adolescents, and families facing challenges in mental and behavioral health. We are currently seeking a Case Manager to play a vital role in our team. This position serves as the primary connection between clients and external resources, ensuring effective leadership and advocacy to foster strong relationships among treatment teams, families, and community partners.

Compensation: $18 - $20 per hour

Key Responsibilities:

  1. Act as the main point of contact for families and external partners, addressing any issues that may arise during the treatment process.
  2. Provide education to families and stakeholders regarding the treatment philosophy, approach, and expectations for parental involvement.
  3. In collaboration with the treatment team, parents, and external partners, document and communicate sensitive information in accordance with policies and procedures.
  4. Ensure that all documentation related to services, including consents, releases, intake procedures, and discharge reports, complies with federal, state, and agency regulations.
  5. Maintain accurate records of case management activities.
  6. Verify financial and clinical eligibility for services prior to their delivery, assisting families in making financial arrangements and connecting them with available resources.
  7. Ensure timely billing for case management services and communicate necessary eligibility information to the Accounting/Payroll technician.
  8. Lead and coordinate activities related to referrals, admissions, assessments, and discharges, adhering to organizational policies.
  9. Oversee the design, implementation, and documentation of treatment plans, ensuring alignment with treatment objectives and progress.
  10. Facilitate collaboration among treatment teams and external entities to address issues impacting client care.
  11. Coordinate with guardians and community organizations to ensure a unified approach to treatment.
  12. Lead meetings, intake sessions, and case conferences, ensuring thorough evaluation and revision of treatment plans as needed.
  13. Represent the agency in custody hearings and other legal proceedings when required.
  14. In specific service areas, may act as the medication representative, coordinating with psychiatrists to optimize treatment.
  15. Work with management to identify and cultivate new referral sources while strengthening existing relationships.
  16. Participate in training and professional development opportunities to stay informed about trends in mental health services and Medicaid regulations.
  17. Monitor daily service delivery and collaborate with other providers to ensure coordinated treatment efforts.
  18. Maintain accurate records and reports in compliance with legal and organizational standards.
  19. Support strategic goals and revenue cycle management initiatives.
  20. Collaborate with management to balance service efficacy with maximum occupancy in case management.
  21. Guide parents towards educational resources and training for skill development in child management and safety.
  22. Discuss and coordinate transportation and medication management needs as appropriate.

Additional Responsibilities:

  1. Support marketing and branding initiatives.
  2. Engage in special projects or duties as assigned by management.

Qualifications:

  1. Bachelor's degree in a social science field or a related area with relevant experience.
  2. Preferred: Case Management Certification.
  3. Must possess a valid driver's license.
