The Community Services Coordinator (CSC) will oversee designated housing communities, collaborating closely with on-site personnel and the supervisory team. The CSC will actively engage with all team members within their assigned portfolio in a thorough and ongoing manner. This role involves continuous evaluation and management of healthy aging strategies and transition plans in partnership with the Resident Service Coordinators.

The CSC is instrumental in building partnerships with local health and social service organizations to facilitate care management meetings, ensuring that complex resident situations are effectively coordinated across various agencies while addressing the collective needs of residents through evidence-based approaches.

Key Responsibilities

  • Foster a positive and professional rapport with assigned communities and colleagues.
  • Develop and update training materials for new Resident Service Coordinators (RSCs) and maintain the RSC manual.
  • Conduct training sessions for new and existing RSCs on company policies and procedures.
  • Regularly analyze internal financial and performance reports, along with HUD regulatory and RSC programming reports; prepare and submit all HUD reports and budgets for the assigned portfolio.
  • Oversee the completion of annual performance evaluations for team members within the portfolio, including follow-up on development or improvement plans.
  • Review and authorize expense reports, supply orders, and invoices submitted by team members.
  • Manage all payroll functions, including mid-week e-times, final payroll, and tracking of vacation and sick leave.
  • Collaborate with the Director of Resident Services Programs (DRSP) to oversee the collection and updates of all reasonable accommodations and modifications.
  • Work in partnership with colleagues to enhance performance, implement corporate policies, and maintain high housing awareness.
  • Ensure team members accurately maintain resident and program data using AASC Online, including case management and resident engagement.
  • Encourage and motivate team members to develop programs aimed at preventing social isolation and promoting health and well-being.
  • Ensure effective collaboration with local agencies to review discharge plans for seamless transitions back to the community.
  • Facilitate team meetings with community partners for strategic planning.
  • Ensure timely submission of monthly management reports and additional reports as required by HUD and TRM.
  • Assist the DRSP in the recruitment process for RSC positions and participate in hiring decisions.
  • Support team members in cultivating positive tenant relations and organizing resident programs.
  • Attend staff and departmental meetings as required.
  • Be available to work evenings or weekends as necessary.
  • Participate in special projects and provide coverage for TRM sites as needed.

Qualifications

  • Completion of coursework or seminars as outlined in the HUD Handbook to meet Service Coordinator Program requirements, with current certification.
  • Bachelor's degree in Social Work, Gerontology, Psychology, or Counseling, or a related combination of education and experience.
  • A minimum of four years of experience working with elderly individuals or those with developmental challenges.
  • Bilingual proficiency in English and Spanish, both written and spoken.
  • Preferred experience in health education and prevention programs.
  • Familiarity with AASC Online and HUD-provided software is advantageous.
  • Two years of experience with ADP Workforce Now software is preferred.
  • Basic computer skills, including proficiency in Microsoft Office Suite and internet navigation.

Travel Requirements

  • Frequent local travel is expected.
  • Occasional out-of-state travel may be required for conferences and training.
  • A valid California Driver's License and proof of current vehicle insurance are necessary.

COVID-19 Vaccination Requirement:

Full COVID-19 vaccination is required by the first day of employment, with documentation to be submitted to Human Resources.

Physical and Work Environment Requirements:

This position may involve prolonged periods of sitting and using hands for computer operation, with occasional standing, walking, and lifting of up to 20 lbs. Exposure to various environmental conditions may occur.
