Job Title: Family Support Specialist

First Choice Health Centers is a Federally Qualified Health Center (FQHC) with opportunities in East Hartford, CT

The Family Support Specialist is responsible for conducting parent education home visits under the Parent as Teachers Program. This position requires outreach, recruitment, and comprehensive risk assessments of high-risk parents who may be eligible for maternal, infant, and early childhood home intervention services. Deliverables include implementation of ongoing periodic screenings and evaluations of parents and children during weekly home visitation sessions, individual and group-level parenting education and interventions, and concurrent documentation of contacts, data, and progress reports. The Family Support Specialist provides and coordinates case management and support services to participating families.

Key Responsibilities:
  • Ability to serve an active caseload of 15-20 families and conduct 30 hours of home visits per week
  • Working knowledge and familiarity with parenting issues and problem-solving skills
Requirements:
  • Three years of comprehensive family case management experience required, four or more years preferred
  • Two years of in-home service experience required, three or more years preferred
  • One year of community outreach and recruitment experience required, two or more years preferred
Preferred Qualifications:
  • Associates Degree in Social Services, Human Services, or equivalent education/related-experience required, BA strongly desired
  • Proficiency with Microsoft Windows and popular media devices (Outlook, Word, Excel, iPhones, projectors, laptops, etc.) required
  • Valid CT driver's license, own transportation, and auto insurance required
  • Bi-lingual in English and Spanish strongly preferred, bi-lingual in English and Twi or similar Ghanaian tribal languages; and/or Vietnamese desirable
  • Strong command of verbal and written communication skills, interpersonal skills, assertiveness, and professional boundaries
  • Ability to strategize, plan, and implement individual and group-level education interventions
  • Ability to prioritize multiple workload demands and effectively manage limited time and resources
  • Ability to model and promote positive parenting qualities and behaviors
  • Ability to work independently and as a cooperative member of an interdisciplinary team of professionals within a health care organization
  • Ability to work with a diverse population
  • Flexibility with schedule required
  • EHR experience preferred
