Job Summary

We are seeking a highly skilled and compassionate Health Care Coordinator to join our team at Partnership HealthPlan of California. As a key member of our Care Coordination Department, you will play a vital role in ensuring our members receive the highest quality care and support.

Key Responsibilities
  • Act as the primary point of contact for members, families, providers, and community agencies, providing guidance and support in navigating our programs and services.
  • Collaborate with members, families, providers, and community agencies to identify care coordination needs and develop personalized plans to address them.
  • Conduct thorough assessments to identify care coordination needs and develop targeted interventions to address them.
  • Research and connect members with available internal and external resources, including community agencies and healthcare providers.
  • Communicate effectively with members, providers, and community agencies to ensure timely access to care and services.
  • Document all interactions and interventions in a timely and accurate manner, in accordance with department policies.
  • Support the Care Coordination Department by answering phone calls, determining call urgency, and routing calls appropriately.
  • Provide exceptional customer service and communicate with others in accordance with Partnership HealthPlan of California's standards and expectations.
Requirements
  • High school diploma or equivalent combination of experience and education required; associate's or bachelor's degree preferred.
  • Two (2) years of experience working in a healthcare setting, preferably in case management or care coordination.
  • Ability to use good judgment, take personal initiative, and use discretion in performing job responsibilities.
  • Attention to detail and accuracy.
  • Basic knowledge of medical terminology and coding.
  • Ability to listen actively and practice motivational interviewing.
  • Knowledge of the healthcare industry and basic concepts of managed care.
  • Ability to learn and adapt to new information and technologies.
  • Valid California driver's license and proof of current automobile insurance compliant with Partnership HealthPlan of California's policy.
Preferred Qualifications
  • Bilingual skills in Spanish, Russian, or Tagalog.
  • Experience working with Medi-Cal and other special needs populations.
  • Ability to work effectively with ethnically and culturally diverse populations.
Work Environment

This is a standard cubicle workstation with a shared common area. The successful candidate will be required to sit in front of computer equipment and talk on the phone for a major portion of the workday. The ability to work in a fast-paced environment and maintain courtesy and composure when dealing with internal and external customers is essential.
