Job Summary

We are seeking a highly skilled Resource Specialist to join our team at NorthBay Health. As a key member of our Inpatient Case Management team, you will play a critical role in maximizing the utilization of hospital resources and coordinating post-hospital care for patients.

Key Responsibilities
  • Communicate and coordinate with payers and outside agencies to assist with patient discharge
  • Support Inpatient Case Management accountabilities
  • Demonstrate adherence to department and system policies, procedures, quality assurance, guidelines, and goals
  • Take day-to-day direction for priorities from the Inpatient Case Management Supervisor or designee
Requirements
  • High school graduate or equivalent preferred
  • Experience in utilization management, discharge planning, and/or social work preferred
  • One or more years of experience in a hospital or medical setting required
  • Knowledge of medical terminology required
