We provide geriatric care management, non-medical home assistance, and RN/LVN home health services to clients on a private-pay basis. The Clinical Nurse Care Coordinator (NCC) is responsible for delivering care management services to clients, particularly those with intricate medical requirements who benefit from informed medical oversight.

Key Responsibilities Include:

  • Client Assessments: Conduct thorough evaluations of clients, engaging with them and their families to understand their current care circumstances, identifying risks and strengths, and addressing any concerns regarding abuse or neglect.
  • Care Planning: Formulate detailed care plans tailored to meet client needs and aspirations, including recommendations on care levels, presenting various care options and associated costs, and pinpointing suitable community resources.
  • Care Management: Ensure the quality of services delivered to clients by managing care coordination and effectively communicating the care plan to all team members, including non-medical Home Care Aides, coordinating care for clients with complex medical conditions, conducting home visits, and participating in client medical appointments.
  • Home Health Services: Perform skilled nursing visits that encompass medication management, injections, ostomy and catheter care, g-tube feedings, and other nursing tasks.

Work Schedule: Standard hours are from 8:30 AM to 5:00 PM, Monday through Friday. This role may require working in client residences, remotely, or from designated company offices.

Desired Qualifications:

  • Active California Registered Nurse (RN) license.
  • Minimum of 1 year of paid clinical nursing experience with adult populations, particularly older adults or those with physical and/or mental disabilities.
  • Strong written and verbal communication skills with meticulous attention to detail.
  • Willingness to travel throughout the region as needed, including visits to various office locations.
  • Proficient in technology, including mobile applications and web-based office systems.
  • Ability to work autonomously, manage travel efficiently, handle multiple projects in a dynamic environment, and accurately report mileage, expenses, and billable hours.
  • Valid driver's license, clean driving record, and dependable vehicle.
  • Current CPR certification.
  • Ability to pass a background check, TB test, drug screening, and health assessment.
  • Capability to lift, push, or pull up to 40 lbs. without assistance.

Preferred Qualifications:

  • Bachelor's degree in nursing (BSN) from an accredited institution.
  • Familiarity with physical and mental health care across various settings.
  • Knowledge of care resources available in the region.
