Clinical Care Coordinator

We are seeking a skilled RN Case Manager to join our team at UHS. As a key member of our healthcare team, you will be responsible for managing resource utilization and clinical outcomes for patients, ensuring seamless transitions and optimal care.
Responsibilities- Develop and implement individualized care plans to meet the unique needs of patients
- Collaborate with healthcare providers to ensure timely and effective care
- Manage resource utilization and optimize patient outcomes
- Participate in denial management and appeals processes
- Provide education and support to patients and families
- 2 years of previous acute care RN experience required
- Bilingual preferred (Spanish)
- Experience in healthcare preferred
- Must demonstrate commitment to UHS's Compliance Program and Code of Conduct
- Licensed in the State of Texas
- Current BLS certification required
- Registered Nurse currently licensed in the State of Texas or part of a compact state license
