Position Overview:

The LVN Case Coordinator plays a vital role in the healthcare team, ensuring that patient admissions and transfers are managed effectively and in accordance with established medical necessity criteria. This position is essential in promoting optimal patient care and resource utilization within the healthcare system.


Key Responsibilities:

  • Facilitates all patient admissions, including inter-facility transfers, while ensuring compliance with admission criteria and medical necessity.
  • Utilizes established screening criteria to assess patient needs and coordinate appropriate care plans.
  • Collaborates with interdisciplinary teams to enhance patient care and participate in quality improvement initiatives.
  • Conducts thorough evaluations of patient cases to identify needs, resources, and care objectives.
  • Implements and monitors care plans, adjusting as necessary to meet patient goals.
  • Provides education and support to patients and families regarding care plans and discharge processes.
  • Maintains accurate documentation of patient interactions and care plans in accordance with regulatory standards.
  • Engages in ongoing professional development and education related to case management practices.

Qualifications:

  • Graduation from an accredited vocational nursing program.
  • Current Texas Vocational Nurse license.
  • Proficiency in both English and Spanish is required.
  • Experience in case management and utilization management is preferred.
  • Three to five years of clinical experience in a hospital or acute care setting is strongly preferred.
