Position Overview:
The Senior RNAC - MDS Team Leader will oversee the MDS team, ensuring the precise and timely execution of MDS assessments and the coordination of the RAI process. This role involves collaboration with an interdisciplinary team to create, update, and maintain a thorough care plan.
Key Responsibilities:
- Lead the RAI process, which encompasses the Minimum Data Set (MDS) and care area assessment process, along with care plan development, implementation, and evaluation.
- Supervise the baseline care plan and ensure the comprehensive care plan meets regulatory standards.
- Assess the skilled level of care for Medicare residents and manage necessary Medicare-specific documentation, including physician certification.
- Coordinate care with case managers for residents utilizing managed care and alternative payment models.
- Participate in quality assessment and assurance (QAA) and quality assurance & performance improvement (QAPI) committees.
- Collaborate with the Admission Director and physicians to secure accurate and complete documentation.
Qualifications:
Applicants must possess a current, unencumbered active RN license in the state or a compact state. A minimum of 2 years of MDS experience is required. Familiarity with current state and federal regulations, including sections of Appendix PP related to Resident Assessments and Comprehensive Resident-Centered Care Plans, as well as Medicare and Medicaid Eligibility and Entitlement, is essential.
