Assessment Coordinator

Belvidere Healthcare Center, a part of the Bear Mountain Healthcare family, is seeking a skilled Assessment Coordinator to join our team.
The Assessment Coordinator plays a critical role in maximizing facility reimbursement by working closely with the clinical team. This position requires strong organizational and communication skills, as well as the ability to prioritize tasks and manage multiple projects simultaneously.
Key Responsibilities:- Generate UDA's (nursing assessments) prior to MDS ARD, with a focus on timely completion.
- Ensure all UDA's are filled out and locked, and that they are completed in a timely manner.
- Monitor compliance with current and upcoming UDA's, and communicate with leadership if there are any issues.
- Work with the MDS Coordinator to ensure that MDS assessments have supporting documentation.
- Perform MDS assessments as delegated, and assist with ensuring documentation is present for behavior add-ons.
