The Hospice Case Manager (RN) plays a pivotal role within our clinical team, exemplifying leadership in nursing care delivery. This position is responsible for providing professional nursing services to patients in alignment with established care plans. The Hospice RN ensures high-quality care during patient interactions and thorough documentation practices. Compliance with agency policies and procedures is paramount. The RN will deliver care that meets all relevant professional standards and adheres to state and federal regulations.

  • Conducts comprehensive assessments of patient status, encompassing physical, psychosocial, and environmental factors
  • Ensures adherence to federal and state regulations governing hospice services through consistent policy application
  • Develops individualized care plans based on assessment data, revising them at least bi-weekly or as necessary
  • Implements skilled interventions aimed at achieving realistic patient outcomes within designated timeframes
  • Executes interventions within the nursing scope of practice
  • Maintains accurate and thorough documentation of all clinical data, including admissions, physician orders, and skilled nursing visits
  • Demonstrates technical nursing competencies in accordance with legal practice standards
  • Provides tailored education to patients and families, adjusting as necessary
  • Conducts clinical chart reviews as needed
  • Assumes on-call responsibilities as assigned
  • Conducts patient visits efficiently, ensuring timely documentation per branch policies
  • Evaluates patient eligibility for admission and assists in personnel assignments
  • Collaborates with physicians to develop and adjust care plans based on patient needs
  • Prepares clinical progress notes and reviews documentation regularly to ensure compliance with hospice standards
  • Ensures that necessary medical supplies and equipment are available for assigned patients
  • Adheres to patient care policies and implements patient care protocols
  • Provides supervisory and educational visits for staff on assigned cases, reporting findings to management as needed
  • Facilitates orientation and training for care teams to maintain high-quality care standards
  • Ensures documentation by home health aides is complete and appropriate
  • Complies with state licensure requirements and agency protocols
  • Recommends policy improvements to enhance patient service quality and operational efficiency
  • Completes additional assignments as requested to support company objectives

  • Graduate of an accredited nursing program, with a Bachelor's degree preferred
  • Minimum of one year of nursing experience in hospice or post-acute care settings
  • Management experience is not required, but supervision of hospice aides is expected
  • Current Registered Nurse licensure in good standing within the state(s) of practice
  • Current CPR certification
  • Valid driver's license and automobile insurance
