Job Summary
The RN Unit Care Coordinator is a key member of the Life Care Center of America team, responsible for overseeing the delivery of high-quality patient care on their respective unit. This role requires strong leadership and interpersonal skills, with a focus on coordinating and managing patient care services.
Responsibilities
- Supervise and direct the daily functions of unit nurses and CNAs to ensure effective patient care.
- Chart patient information accurately and in a timely manner, maintaining confidentiality and adhering to regulatory requirements.
- Provide and coordinate patient care services, promoting the highest practicable physical, mental, and psychosocial well-being of patients.
- Administer medication as ordered by a physician, ensuring accuracy and attention to detail.
- Respond to emergency situations related to patient and staff safety, demonstrating leadership and critical thinking skills.
- Coordinate patient care plans and services, working collaboratively with interdisciplinary teams.
- Exhibit excellent customer service and a positive attitude towards patients, families, and colleagues.
- Assist in the evacuation of patients, demonstrating dependability and a commitment to patient safety.
- Concentrate and use reasoning skills and good judgment, making informed decisions in a fast-paced environment.
- Communicate and function productively on an interdisciplinary team, promoting a culture of collaboration and respect.
- Perform physical demands of the job, including sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during working hours.
- Read, write, speak, and understand the English language, ensuring effective communication with patients, families, and colleagues.
Requirements
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State, with a commitment to maintaining an active Registered Nurse (RN) license in good standing throughout employment.
- One (1) year geriatric nursing experience preferred, with a focus on patient care coordination and management.
- CPR certification upon hire or obtain during orientation, with a commitment to maintaining current certification during employment.
Preferred Qualifications
- Advanced knowledge in the field of geriatric nursing, with a focus on patient care coordination and management.
- Ability to make independent decisions when circumstances warrant such action, demonstrating leadership and critical thinking skills.
- Knowledge of practices and procedures, as well as laws, regulations, and guidelines governing functions in the post-acute care facility.
- Ability to implement and interpret programs, goals, objectives, policies, and procedures of the department, promoting a culture of excellence and quality care.
