Patient Navigator

Job Summary:
We are seeking a highly skilled Patient Navigator to join our Ambulatory Care Management team at Lancaster General Health. The successful candidate will be responsible for coordinating care plans, providing advocacy to patients, and developing linkages for health and social needs.
Key Responsibilities:
- Coordinate care plans and complete tasks to achieve social/behavioral care plan goals
- Act as a liaison between the Emergency Department, specialists, community resources, and managed care/insurance plans
- Engage in coaching patients and caregivers on health literacy
- Forge partnerships with local service organizations and community resources
- Enter and maintain electronic records, compile reports, and complete other program documentation
Requirements:
- High School Diploma or General Equivalency Degree (GED) with at least one year of experience in the social services and/or medical field
- Strong understanding of medical terminology, case management, and/or outreach services
- Exceptional organizational and interpersonal skills, with attention to detail
- Ability to work collaboratively in a team, manage multiple priorities, and exercise sound administrative and clinical judgment
Preferred Qualifications:
- Licensed Practical Nurse (LPN), Paramedic/Emergency Medical Technician, or other licensed health care practitioner
- Bilingual (proficiency in the Spanish language)
- Prior acute hospital experience or ambulatory practice experience
