Nurse Navigator

Medix is seeking an experienced Orthopedic Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for coordinating care for patients undergoing total joint replacement surgeries.
Responsibilities:
- Coordinate care for patients undergoing total joint replacement surgeries
- Lead joint education classes for patients (both individual and group sessions)
- Provide preoperative education and clearance for patients
- Monitor patient progress, ensuring follow-ups post-surgery and during discharge planning
- Participate in community outreach and marketing efforts, including senior center presentations and distributing educational materials
- Collect and track metrics related to patient outcomes
- Work closely with a multidisciplinary team including total joint surgeons, RNs, PAs, and OR staff
Requirements:
- RN License - PA or compact
- Experience in PACU, orthopedic nursing, or as a nurse educator preferred
- Prior experience with EPIC or other EMR systems
- Background in total joint procedures highly preferred
- Strong organizational skills, bedside manner, and adaptability
- Experience in preoperative and postoperative care coordination
- Ability to work autonomously and help build out a growing program
