Job Summary:

As a Clinical Social Worker at Lancaster General Health, you will play a vital role in providing psychosocial care to patients and their families. You will work as part of an interdisciplinary team to assess the needs of patients and develop treatment plans that address their social, emotional, and financial well-being.

Key Responsibilities:

Conduct comprehensive psychosocial assessments of patients and report findings to families, physicians, and other healthcare providers as appropriate.

Facilitate social service contacts and develop treatment plans in coordination with social service resources.

Provide counseling and support to patients and their families to help them understand and navigate the healthcare system.

Develop and maintain relationships with community agencies and facilitate communication between these agencies and patients, families, and clinical personnel.

Act as a patient advocate and investigate and report adverse occurrences.

Requirements:

Master's degree in Social Work from an accredited program.

Two years of experience in social work, preferably in a hospital setting.
