Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as an Integrated Care Specialist. In this role, you will work collaboratively with an interdisciplinary team to provide comprehensive care to patients with complex needs.

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough assessments of patients' physical and mental health, substance use history, and social determinants of health.
  • Develop Individualized Care Plans: Develop personalized care plans that integrate behavioral health, addiction medicine, and primary care interventions.
  • Administer Medications and Provide MAT: Administer medications and provide medication-assisted treatment (MAT) for substance use disorders, monitoring patients' responses to treatment.
  • Collaborate with Interdisciplinary Teams: Work collaboratively with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care.
  • Offer Evidence-Based Counseling and Psychoeducation: Provide evidence-based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management.
  • Conduct Regular Follow-Up Visits: Conduct regular follow-up visits to monitor patients' progress, provide ongoing support, and adjust treatment plans as needed.
  • Maintain Accurate Medical Records: Maintain accurate and up-to-date medical records and documentation, adhering to regulatory guidelines and best practices.
  • Participate in Continuing Education: Actively participate in continuing education and professional development opportunities to stay current with best practices and industry developments.
Qualifications
  • Valid Nursing License: Possess a valid nursing license (RN or LPN) in Florida.
  • Experience in Integrated Healthcare: Have experience working in integrated healthcare settings, behavioral health, addiction medicine, and/or primary care.
  • Strong Understanding of Mental Health and Addiction: Possess a strong understanding of mental health disorders, addiction treatment modalities, and physical health.
  • Excellent Assessment and Critical Thinking Skills: Demonstrate excellent assessment and critical thinking skills to provide high-quality care.
  • Ability to Work Independently and Collaboratively: Possess the ability to work independently and collaboratively within an interdisciplinary team.
  • Outstanding Communication and Interpersonal Skills: Demonstrate outstanding communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
  • Compassionate and Patient-Centered Approach: Possess a compassionate and patient-centered approach to care, prioritizing the needs and well-being of patients and their families.
