Care Coordinator - Speech and Language

We are seeking a skilled Speech Language Pathologist to join our team as a Care Coordinator - Speech and Language. In this role, you will work closely with patients to develop and implement individualized care plans, addressing their unique needs and goals.

About the Role:

  • Develop and implement patient care plans in partnership with patients, representatives, and caregivers.
  • Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders as ordered by physicians.
  • Coordinate care with other healthcare professionals to ensure comprehensive support for patients.

Requirements:

  • Masters or doctoral degree in speech-language pathology from an accredited college or university.
  • Active state license as a speech-language pathologist.
  • One year of experience in acute care or rehabilitation setting; home health preferred.

Salary Estimate: $85,000 - $110,000 per year, depending on experience.
