As a Patient Care Coordinator with Ascension Seton, you will be part of a team that is dedicated to providing compassionate, personalized care to our patients.

Job Summary

This role is responsible for coordinating patient care services, ensuring seamless transitions between departments, and promoting a positive patient experience.

The ideal candidate will have excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities

  • Coordinate patient admissions, discharges, and transfers
  • Manage patient flow and ensure efficient use of resources
  • Communicate effectively with patients, families, and healthcare teams
  • Monitor and report on patient satisfaction and quality metrics
  • Collaborate with interdisciplinary teams to develop and implement patient care plans

Requirements

  • Bachelor's degree in Nursing or related field required
  • 2+ years of experience in patient care coordination or a related field
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
  • Strong problem-solving and analytical skills
