Care Coordinator Specialist Job Description

Job Summary

The Care Coordinator Specialist will play a vital role in ensuring the seamless navigation of patients through complex healthcare environments. This hybrid role is located in Austin, Texas, and requires the candidate to reside in the greater Austin metropolitan area.

This position functions autonomously and collaboratively with other healthcare team members. The successful candidate will be responsible for providing inbound/outbound call support, addressing access issues, scheduling appointments, and navigating the referral process.

Key Responsibilities
  • Collaborate with the patient's care team, including primary care physicians, specialists, virtual care providers, home care teams, and hospital staff, to optimize clinical outcomes.
  • Review patient progress, coordinate communications, and facilitate scheduling of appointments as needed.
  • Assist with the referral process, track outcomes, and educate patients and family members on diagnosis, treatments, procedures, and medications.
  • Support transitions of care activities, enhance patient experience, and participate in continuous quality improvement projects.
  • Maintain awareness of key performance indicators, coordinate interventions, and attend staff meetings and education offerings.
Requirements
  • Graduation from an accredited school of nursing with a minimum of an Associate's Degree.
  • Two years of nursing experience in an ambulatory clinic and/or hospital setting.
  • PREFERRED: Bachelor of Science in Nursing (BSN), One year of Case Management or Navigation experience in a navigation center and/or ambulatory clinic setting.
Qualifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure.
  • Basic Life Support (BLS) obtained through approved American Heart Association or American Red Cross.
Estimated Salary Range:

$65,000 - $85,000 per annum, depending on qualifications and experience.
