We're seeking a highly skilled and compassionate Senior Care Coordinator to join our team at CenterWell. As a key member of our interdisciplinary care team, you'll play a vital role in ensuring our patients receive the highest quality care and support.

Job Summary

The Senior Care Coordinator will work closely with patients, families, and healthcare providers to coordinate and facilitate care across multiple settings. This includes developing personalized care plans, identifying and addressing social determinants of health, and providing education and support to patients and families.

Key Responsibilities

  • Develop and implement care plans that address the unique needs and goals of each patient.
  • Conduct regular assessments to identify potential risks and barriers to care.
  • Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and care.
  • Provide education and support to patients and families on topics such as disease management, medication adherence, and community resources.
  • Identify and address social determinants of health that impact patient outcomes.
  • Lead interdisciplinary team meetings and provide feedback and recommendations for improvement.
  • Maintain accurate and timely records of patient interactions and care plans.
  • Adhere to all relevant policies and procedures, including those related to confidentiality and HIPAA.
