Key Responsibilities

The social worker will implement care standards for social work services within the home health and hospice context.

Core Functions Include:

Evaluating the psychosocial conditions of clients in relation to their health and surroundings, and sharing insights with the healthcare team.

Conducting social assessments and devising intervention strategies based on findings.

Maintaining accurate clinical documentation for all clients referred for social work services.

Providing information and referral assistance to clients and their families regarding community resources.

Acting as a liaison between clients, families, and community organizations.

Collaborating with organizational staff to enhance client care.

Establishing and nurturing partnerships with public and private agencies to support client needs.

Participating in the development of comprehensive care plans and case discussions as necessary.

Assisting the medical team in recognizing significant social and emotional factors impacting health issues.

Contributing to discharge planning processes.

Coordinating volunteer training, ongoing education, and performance evaluations.

Assigning volunteers to support hospice families and organizing their visits.

Overseeing bereavement support programs.

Collaborating with leadership to serve as a contact for community foundation initiatives.

Ensuring compliance with documentation standards for social services in line with regulatory requirements.

Performing additional duties as assigned by supervisory personnel.

Qualifications

A Bachelor's degree in Social Work is required.

Must hold a valid Social Worker license in the relevant states.

A minimum of one year of experience in a healthcare setting is preferred.

Strong verbal and written communication skills, along with excellent organizational abilities.

Basic proficiency in computer applications is necessary.

Preferred Qualifications:

A Master's degree in Social Work is advantageous.

Experience in Home Health and Hospice settings is desirable.
