Key Responsibilities

The Community Support Specialist is tasked with implementing care standards for social work services within the home health and hospice environment.

Essential Functions:
  • Evaluate the psychosocial status of clients concerning their health conditions and surroundings, relaying insights to the nursing staff.
  • Conduct social assessments and develop intervention strategies based on evaluation results.
  • Maintain comprehensive clinical records for all clients referred to social work.
  • Provide information and referral services to clients and their families regarding environmental needs.
  • Facilitate communication between clients, families, and community organizations.
  • Promote agency programs and events within the local community.
  • Act as a liaison between clients, families, and community resources.
  • Foster collaborative relationships with organizational personnel to enhance client care.
  • Develop and maintain partnerships with public and private agencies to support clients and staff.
  • Participate in the formulation of comprehensive care plans and case discussions as necessary.
  • Assist the medical team in recognizing significant social and emotional factors impacting health issues.
  • Engage in discharge planning activities.
  • Coordinate volunteer training, ongoing education, and annual assessments.
  • Assign volunteers to hospice families and oversee their visits.
  • Manage the bereavement program.
  • Collaborate with the agency director as the primary contact with the foundation department at the local hospital.
  • Oversee event planning and execution.
  • Ensure accurate documentation of social services in clinical records, adhering to state, federal, and Joint Commission standards.
  • Perform additional duties as assigned by the Clinical Director/Supervisor.
Qualifications

  • Bachelor's degree in Social Work
  • Licensed Social Worker in Minnesota and North Dakota
  • Minimum one year of experience in healthcare
  • Strong verbal and written communication skills, along with excellent organizational abilities
  • Basic computer proficiency
Preferred Qualifications:
  • Master's degree in Social Work - salary commensurate with experience
  • Experience in Home Health and Hospice settings

Compensation:

Pay Range: $27.94/hour
