Position Overview:

Role Title: Intensive Support Coordinator (ISC)

Job Summary: Provide essential resources and support to individuals with intellectual and developmental disabilities, along with their families and guardians, to ensure they are integrated into society, live as independently as possible, and function at their highest potential.

Key Responsibilities:

  • Adhere to all policies and procedures established by the organization, Georgia Department of Community Health, and Home and Community Based Services (HCBS) regulations
  • Conduct evaluations and assessments to address the needs of individuals served
  • Assist individuals in applying for financial aid, residential planning, vocational, recreational, educational needs, healthcare, in-home supports, day services, legal, nutritional, transportation, social, and other related services and resources.
  • Advocate for services that promote the individual's success
  • Maintain comprehensive case records
  • Prepare, update, and monitor Care Path Plans
  • Support individuals in participating in and directing the person-centered planning process
  • Coordinate meetings with individuals, families, guardians, and interdisciplinary team members as necessary
  • Establish and maintain positive relationships with individuals, families, guardians, state officials, and team members
  • Document work and billing time in accordance with company policies
  • Track, monitor, and input specified data points
  • Report any suspected abuse, neglect, or exploitation immediately to a supervisor or department head

Qualifications:

  • Bachelor's degree in human services, social work, sociology, or psychology
  • Valid driver's license required
  • Maintain auto insurance
  • Minimum of 2 years of experience in a similar role
  • Willingness to travel up to 75%

Knowledge and Skills:

  • Basic understanding of the state's case management system
  • Basic understanding of the state's waiver programs
  • Ability to work collaboratively while achieving individual performance goals
  • Effective verbal and non-verbal communication skills with a diverse audience of stakeholders
  • Proficiency in using technology tools in daily tasks
  • Familiarity with HCBS regulations, person-centered planning, and related best practices
