Nurse Care Coordinator

Pony Bird, Inc. is seeking a highly skilled Registered Nurse to join our team in Hillsboro. As a key member of our healthcare team, you will be responsible for providing exceptional care to individuals with intellectual and developmental disabilities.
Key Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of our clients.
- Conduct regular assessments and evaluations to ensure the highest level of care and support.
- Delegate tasks to unlicensed caregivers and provide guidance and oversight as needed.
- Coordinate and oversee the Level 1 Medication Aide training program.
- Provide community RN coverage to assigned locations and maintain a cohesive nursing team.
- Establish evidence-based nursing practices and review lab results to ensure accurate care.
- Monitor medication administration records and review records for accuracy and employee compliance.
Requirements:
- Minimum of three years of registered nursing experience and one year of supervisory experience.
- CPR/AED certification and a valid Missouri Chauffer's License.
- Certified Level 1 Medication Aide Instructor or obtain upon hire.
- Certified DMH Community RN or obtain upon hire.
- Experience and knowledge of care principles affecting individuals with intellectual and developmental disabilities.
- Supervisory and organizational skills, excellent interpersonal and communication skills, and ability to maintain confidential information.
