We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health. As a Care Coordinator, you will play a critical role in providing exceptional care to children, youth, and families with significant behavioral health needs.
Key Responsibilities- Develop and implement individualized care plans that address the unique needs of each child, youth, and family.
- Collaborate with community agencies and resources to ensure seamless service delivery and coordination.
- Engage and build positive relationships with children, youth, and families to support successful integration of care plans.
- Provide education and support to families on available resources and services.
- Document and maintain accurate records of service delivery and outcomes.
- Participate in program evaluation and quality improvement initiatives.
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
- Three years of experience with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
- Knowledge and experience in Hi-Fidelity Wraparound preferred.
- Two years of experience in a coordinated supportive services or care coordination role is preferred.
- Experience working with people with autism spectrum disorders and developmental disabilities is preferred.
