Clinical Care Manager

Spectrum Health Systems is seeking a highly skilled Clinical Care Manager to join our team. As a Clinical Care Manager, you will be responsible for collaborating with team members to implement measures that decrease episodic care and meet quality outcomes.
Key Responsibilities:
- Supervise a team of care coordinators to help MassHealth members connect to the appropriate care.
- Work as part of a multidisciplinary team consisting of Behavioral health, Medical, Recovery support staff, to help navigate and coordinate care for clients exhibiting complex health issues.
- Participate in meetings with other Community partners, and stakeholders.
- Support care coordinator in advocating for client treatment.
- Develop a person-centered treatment plan with each Engaged Enrollee, with input from Care Team members and other stakeholders as well as updating the treatment plan according to required timeframes.
- Supervise Care Coordinators in supporting Engaged Enrollees during care transitions including attendance at discharge planning meetings, face to face meetings post discharge, and facilitating enrollee participation in those services.
- Conduct weekly supervision with assigned care coordinators.
Requirements:
- Masters degree with first level license (RN, LICSW, LMFT, LMHC, licensed psychologies, or LADAC-I); or Master's degree in a related field and at least two (2) years of relevant post-graduate experience; or Master's degree in an unrelated field and at least (5) years of relevant post-graduate experience.
- Strong skills in the areas of communication, follow through, collaboration, and customer service.
- Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
- Excellent time management and organizational skills
- Strong Computer skills, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
- Must hold a valid drivers' license. Must have access to an operational and insured vehicle
