Key Responsibilities
  • Collaborate with Primary Care Providers to coordinate and manage the care of vulnerable patients, ensuring optimal care and outcomes.
  • Develop and implement comprehensive care plans for patients with complex needs, including those admitted to and discharged from the hospital, emergency room, and other healthcare facilities.
  • Provide direct patient care within the nursing scope of practice, including patient assessment, medication management, and education.
  • Communicate effectively with the care team, patients, and families to ensure seamless care coordination and patient engagement.
  • Document patient care, progress, and outcomes in the Electronic Health Record (EHR) in accordance with health center and nursing standards.
Requirements
  • Registered Nurse License in Massachusetts
  • 1-2 years of clinical experience
  • Current CPR certification
  • COVID-19 booster and annual flu vaccine required
