About the Role:
Bear Mountain Healthcare is seeking an experienced MDS Director to join our team. As a key member of our clinical leadership team, you will be responsible for ensuring the highest quality of care for our patients.
Key Responsibilities:
- Completing accurate assessments, MDS & care plans as assigned
- Initiating care plans and supporting activities as assigned
- Creating and distributing monthly care plan calendars in a timely fashion
- Maintaining and updating care plans and assessments
- Monitoring and auditing clinical records, ensuring accuracy & timeliness
- Ensuring compliance with MDS / Medicare protocols and policies
- Informing DON and ADON of persistent issues related to all non-compliant documentation
Requirements:
- Graduate of an accredited school of nursing with current LPN/RN licensure in Massachusetts
- One year of Long-term care experience
- Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resources utilization, and/or case management is highly desirable
